Adrenal disorder care in Fresno, CA
The two adrenal glands sit above the kidneys and make cortisol, aldosterone, adrenaline and some sex hormones. Too much cortisol causes Cushing's syndrome, with weight gain around the middle, a rounded face, thin skin, muscle weakness and high blood sugar. Too little causes Addison's disease or adrenal insufficiency, with fatigue, weight loss, low blood pressure and salt cravings. Other adrenal problems include aldosterone excess that drives hard-to-control blood pressure, adrenal nodules found on scans, and rare tumors such as pheochromocytoma.

Adrenal testing is easy to get wrong, which is why these conditions are often missed or over-diagnosed. We use the correct, properly timed tests: morning cortisol, ACTH, dexamethasone suppression, late-night salivary cortisol, 24-hour urine studies, aldosterone and renin, and stimulation tests when needed. We interpret them in the context of your medications and symptoms, and we order imaging only when it will change the plan.

Treatment depends entirely on the cause. Adrenal insufficiency is managed with carefully dosed hormone replacement and sick-day rules you can rely on. Cushing's and aldosterone problems may be treated with medication or referred for surgery, and we coordinate closely with surgeons and your other doctors. Adrenal nodules that do not make hormones are followed safely over time. Whatever the diagnosis, you get a clear explanation and a long-term partner in managing it.

Adrenal fatigue is not a recognized medical diagnosis, but the symptoms people describe are real. We test properly for true adrenal insufficiency and for the other causes of persistent fatigue, such as thyroid disease, sleep apnea, anemia, low testosterone or depression, so you get an answer that leads somewhere.
Most adrenal nodules found by chance are benign and do not make hormones, but every one needs a hormone workup and a look at the imaging characteristics. We do that evaluation and set up the right follow-up, which is often just periodic monitoring.
It depends on the source of the excess cortisol. Medication-related Cushing's is managed by adjusting the medication. A pituitary or adrenal tumor is usually treated surgically, with medication to control cortisol before or after. We make the diagnosis and coordinate the team.
Yes. Primary aldosteronism is one of the most common and most missed causes of hard-to-control blood pressure. A simple blood test screens for it, and treatment can bring blood pressure under control and protect your heart and kidneys.
People on cortisol replacement need to increase their dose during illness, injury or surgery to avoid an adrenal crisis. We teach every patient a written plan, prescribe emergency injectable hydrocortisone and recommend a medical alert bracelet.
